Job Description:

Chapa-De, a non-profit community health clinic, has an opportunity for a Clinical RN at our Grass Valley clinic.

POSITION SUMMARY:

The Clinical RN serves as the hub and team leader for all patient care team members on their Pod or team. The Clinical RN sees patients independently, within their nursing scope, and they assist the MA or provider with complicated cases. This position reports to the Director of Nursing or RN Clinical Coordinator and works as part of a team with the clinic providers, and other clinic support staff to provide quality, coordinated care to patients. In addition, the Clinical RN provides triage and patient education. This position supervises the LVNs, and MAs within their Pod.

DUTIES AND RESPONSIBILITIES:

  1. Leadership and Supervision of nursing care in their Pod.
  2. Coordinate the daily patient care delivery within their pod or team
  3. Assist with clinic flow by performing pre-visit assessment for new patients, medication reconciliation, setting the agenda for the visit, and providing patient education.
  4. Perform/direct patient care services including, but not limited to, admission interview, accurate documentation and timely patient flow.
  5. Assures accurate medication reconciliation for complex patients on multiple medications
  6. Triage patient telephone calls utilizing knowledge of professional nursing theory and practice.
  7. Act as to resource for pod team members.
  8. Assist physician/healthcare provider with exams, treatments, procedures and testing as directed. Ensures appropriate consent forms are obtained prior to the above
  9. Provide direct one-on-one patient education and/or Health Coaching
  10. Refill medications by protocol.
  11. Receive, prioritize and respond to incoming calls as appropriate
  12. Coach and mentor clinical team members to build or enhance capability and competency.
  13. Assist with coordination of care to other Chapa-De services, including dental care, behavioral health, diabetes education, etc.
  14. This position requires that the candidate be able to perform duties with minimal supervision, be highly motivated and flexible to address the needs of patients, families, and members of the health care team.
  15. The Clinical RN will be responsible for verifying MA medications/orders prior to administration.
  16. Understand and promote the mission, vision, and values of Chapa-De both in the work place and in the community.
  17. Maintain compliance with all state and federal laws and regulations, as they pertain to position including; HIPAA, Scope of Practice, Accreditation Standards, OSHA, and the Agency’s Policies & Procedures.
  18. Other duties as assigned.

 

 

Experience and Skills:

QUALIFICATIONS:

  1. RN License, current and in good standing with the state of California, BSN Preferred.
  2. A minimum of two years current nursing experience.
  3. Previous Primary Care Clinic experience is strongly desired.
  4. Computer literacy in Microsoft Word & Excel and experience with EHR required.
  5. Sensitive to the health and cultural needs of Native Americans and vulnerable populations.
  6. Ability to work independently, when needed and collaboratively with other members of the health care team.
  7. Current CPR Certificate.
  8. Current and in good standing; CDL, vehicle insurance, and DMV printout.
