Job Description:

Chapa-De Indian Health a non-profit community health center has an opportunity for a part-time Physician or Family Nurse Practitioner.  Tentative Schedule will be Monday through Wednesday from 5 PM to 8 PM and Thursday from 1 PM to 8 PM.

The Staff Physician will be responsible for providing primary health care services consistent with accepted medical practices, training, accreditation standards and the philosophy of Chapa-De Indian Health Program.


  1. Provide direct medical services to Program patients. Scope of work includes adult & pediatric primary care, chronic illness and disease management minor surgeries and procedures, and all other areas of family medicine.
  2. Interdepartmental medical consultation as requested.
  3. Clinical supervision of mid-level staff, resident physicians, FNP/PA and medical students as needed.
  4. Participate in the development of medical services, including protocols, standard procedures, and management of patient care related activities.
  5. Serve as liaison with the local medical community. Initiate appropriate specialty referrals.
  6. Provide leave coverage, if necessary, between the Auburn and Grass Valley sites as agreed upon with the Medical Administrator.
  7. Advise Medical Administrator of any problems that can affect the efficiency of the Medical Department.
  8. Provide required Federal and State Ambulatory Patient Care Data on appropriate forms and follow Indian Health Service health care maintenance guidelines.
  9. Record patient chart entries in Subjective, Objective, Assessment, and Plan (SOAP) format.
  10. Complete appropriate documentation necessary for Data and Billing Departments.
  11. Advise nursing staff of appropriate response to patient phone calls.
  12. Other duties and responsibilities as assigned.

Experience and Skills:

Qualifications for a Family Practice Physcian:

  1. Licensed Physician; Board Certified in Family Practice or Internal Medicine with Pediatric Specialty.
  2. Two years clinical experience in the direct delivery of primary care.
  3. BLS certification required.
  4. Sensitive to American Indian culture and health needs. Prior experience in cross-cultural medicine desirable.

Qualifications for a Family Nurse Practitioner:

  1. Fifth year college or university program certificate; or two to four years related experience and/or training; or equivalent combination of education and experience.
  2. Current CA Nurse Practitioner license.
  3. Current DEA certificate.
  4. Current CPR certificate.
  5. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations.  Ability to write reports, business correspondence, and procedure policies.  Ability to effectively present information and respond to questions from groups of managers, clients, customers, and general public.


  1. Current and in good standing CDL, vehicle insurance and DMV printout required.
